REFERRALS & ELIGIBILITY
California is providing new services for community members in Kings and Tulare counties who have Medi-Cal insurance with Anthem Blue Cross, CalViva Health, Health Net, or Kaiser Permanente.
If you or your community member qualifies – they may be eligible to receive these services at no charge:
• Help to connect to health and social services
• Home-delivered meals
• Support with finding and keeping housing
• Help to access mental health or substance use care
• Help with transportation
• Modifications to be able to stay in their home
• Respite and personal care in the home

Kings County: Call 1-877-410-8813 or complete an application.

Identify the condition or situation that qualifies them for ECM or Community Supports.
These can range from homelessness and complex medical conditions to substance use disorders, severe mental illness, foster care, or recent incarceration.
