This case study describes how organizations in York, Pennsylvania collaborated across healthcare, behavioral health, criminal justice, and community services to create an “ecosystem of care” for individuals involved in the criminal legal system who also have complex health and social needs. The partnership aimed to reduce emergency department use, prevent further criminal legal involvement, and improve physical and behavioral health outcomes through coordinated, cross-sector support.
