ABOUT
This website was created by CalAIM providers, Medi-Cal managed care plans, and PATH Collaborative Planning and Implementation facilitators for CalAIM providers and referring partners in Kings and Tulare counties. It was developed to curate and centralize information to connect and equip providers who are building systems of care to serve the underserved in their communities, making the best use of community resources.

About the PATH Collaboratives Serving Kings and Tulare Counties
The California Department of Health Care Services funded Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation groups to support the implementation of ECM and Community Supports by building local networks to care for Medi-Cal members with complex needs. The Central Valley PATH Collaborative, facilitated by Transform Health, includes Kings, Fresno and Madera counties; the Central Valley facilitators may be reached at CVpath@transformhc.com. The Tulare PATH Collaborative, facilitated by HC2 Strategies in partnership with IHI, includes Tulare County; the Tulare facilitators may be reached at TulareCalAIM-CPI@hc2strategies.com.
HC2 Strategies is a mission-driven strategy and community health consultancy with a national footprint, devoted to helping hospitals, organizations and communities nurture holistic strategies that support community well-being and population health. HC2 Strategies helps health and social services teams unleash their innovation and live to their potential and facilitates. In addition to the Tulare PATH Collaborative, HC2 facilitates the statewide Indian Health PATH Collaborative and seven other regional PATH Collaboratives that serve 23 counties. The organization’s authentic approach defines purpose, creates clarity, and drives unique community health strategies.
